Tax year 2004-05: from 6 April 2004 to 5 April 2005

You need to have following information:

4. National Insurance Number (NI Number) - if you have one
5. Your full employment history - all jobs you had for tax years in question:
6. P60 or/and P45 or/and LAST PAYSLIP - for each job you had
7. Any previous address you had within last 3 years
8. Your nationality
9. When arrived in UK - if you are from abroad
10. Your bank details - if refund to be made to your bank account
If you are self-employed sub-contractor
1. Name
2. Address
3. Date of birth
4. National Insurance Number (NI Number) - if you have one
5. CIS card number or UTR - Unique Tax Reference Number
6. Tax area name - where you receive your tax related letters from
7. Your bank details - if refund to be made to your bank account (Name on the account, sort code, account number)
8. Your income details - can be calculated using your bank statements or invoices (payslips) you had from your contractor

List of tax allowable expenses:
- Cost of goods used - materials
- Car, van and travel expenses - motor expenses (road tax, repairs, insurance etc.), travel expenses - fuel, bus, rail etc.
- Employee costs - if you have employed anybody
- Rent, rates, power and insurance - house rent, business rates (council tax), utilities (gas, water, electricity), business & building insurance
- Repairs & renewals of property & equipment
- Accountancy, legal & other professional fees
- Bank interest, bank charges
- Telephone, stationery & other office cost
- Other expenses (work clothes, staff training & entertainment etc.)
- Motor vehicles - cars, vans
- Plant & Machinery, tools and equipment (e.g. any power tools which you intend to use more than one year);
- Fixtures & Fittings (office furniture, computers, printers etc.)
10. Tax deducted - can be calculated using your bank statements or invoices (payslips) you had from your contractor;
